The catchment shapes the theatre brief

Vidarbha’s districts, plus the bordering belts of Madhya Pradesh, Chhattisgarh and Telangana, funnel their complex surgery into one city. That single fact drives most of the design decisions on a Nagpur theatre project.

Case mix skews complex. Cardiac, oncological, neurosurgical and major trauma work make up a far larger share of the list here than in a comparable tier-2 city, because the simpler cases are absorbed by district hospitals upstream. Those specialities are not served by a baseline theatre. Cardiac and neuro rooms want 25-30 air changes per hour rather than the NABH minimum of 20, a clear floor area nearer 42-50 sq m against 36-40 for general surgery, and a ceiling grid engineered to carry a fixed C-arm, a microscope boom or a three-pendant configuration without a later structural retrofit.

Patients also travel 150-400 km to get here, which compresses scheduling. Lists are long, and the recovery interval — the time a theatre needs to return to its validated particle count after a high-traffic case — stops being a technical footnote and becomes a throughput number. A room validated at the 20 ACH minimum can take 15-20 minutes to recover; the same room at 28 ACH under a correctly sized laminar flow canopy recovers in well under ten. Across an eight-case day that is roughly one additional slot, every day, for the life of the theatre.

Third, the teaching ecosystem changes the occupancy assumption. With AIIMS Nagpur, two government medical colleges and a dedicated cancer institute in the same catchment, theatres carry students, residents and observers. Design heat load and door-opening frequency for eight to ten people in a major OT, not five. Under-counted occupancy is one of the most common reasons a theatre passes commissioning and then fails a re-validation two years later.

HVAC is the Nagpur-specific engineering problem

Panels, doors and flooring are broadly the same anywhere in India. The plant is not.

Nagpur’s summer design dry bulb sits around 45-46°C, five to six degrees above what most vendors quietly assume when they price an OT package from a Mumbai-Pune template. Three consequences follow. Refrigeration capacity per square metre needs to be roughly 15-20 percent higher for the same room. Air-cooled condensers must be selected with ambient derating applied and sited where they are shaded and not recirculating their own discharge — a rooftop plant deck in Nagpur is a materially harsher environment than the same deck in Pune.

The second issue is that Nagpur has two distinct design points, not one. May is hot and dry; August is cooler and saturated. A single-setpoint DX unit sized for the dry peak will hold temperature in July and overshoot humidity badly, and the theatre will sit outside the 50-60 percent RH band that NABH and HTM 03-01 expect. The fix is unglamorous: adequate reheat, proper dehumidification control, and a controls sequence that treats temperature and humidity as separate objectives.

Third, decide the fresh-air fraction deliberately. HTM 03-01 permits recirculation through full HEPA filtration, and in a 46°C ambient the difference between a 100 percent fresh-air design and a sensible recirculation design is a large chunk of running cost for the next fifteen years. That choice belongs in the design review, not in the AHU vendor’s default selection.

Logistics and delivery from Pune

Nagpur is about 700 km from Pune and is one of the best-connected freight destinations in the country — the Mumbai-Nagpur Samruddhi Mahamarg, NH-44 and NH-53, and the Howrah-Mumbai rail trunk all pass through it. A single-theatre panel set is typically one to two trailer loads, flat-packed and sequenced so that the first items off the truck are the first items installed. Road transit is 16-20 hours, and we plan two to three days door to door with contingency.

Site access varies sharply within the city. Greenfield builds around MIHAN and the outer ring have crane access and laydown space, so panels can be delivered in large sections. Established hospitals in Dhantoli, Sitabuldi and the central corridor often have a narrow approach and a service lift as the only vertical route, which means panel modules must be dimensioned to the lift car at design stage. Getting that wrong is discovered on delivery day, and it is expensive.

Compliance and statutory notes

Private facilities register with Nagpur Municipal Corporation under the state’s nursing homes registration framework; government and medical college projects run through the Directorate of Health Services and their own works departments, with tender-driven documentation requirements that are worth confirming before design freeze.

Fire clearance is issued by Nagpur Fire and Emergency Services under the Maharashtra Fire Prevention and Life Safety Measures Act, 2006, read with NBC 2016 Part 4. For theatre work the questions that actually get asked concern panel core material and surface spread of flame, sealing of penetrations between fire compartments, and gas shut-off provisioning consistent with NFPA 99. Specify panel cores with the fire classification you will need at NOC stage rather than substituting later.

Because a large share of the Vidarbha catchment presents under MJPJAY and Ayushman Bharat PM-JAY, empanelment infrastructure criteria are a live design input in Nagpur more than in most Maharashtra cities. Why does Nagpur's summer heat change the OT design?

Because the air handling plant, not the panels, does the compliance work.
