Thane is a hospital market in its own right

Thane city alone carries well over two million residents, and the district around it — Kalyan-Dombivli, Ulhasnagar, Ambernath and Badlapur, Bhiwandi, Mira-Bhayandar — pushes past eleven million. That is a population larger than most Indian states, and for three decades it exported its complex surgery. A patient on Ghodbunder Road or in Dombivli with a cardiac, oncology or joint replacement indication travelled to Parel, Bandra or Andheri to have it done.

That flow is reversing, and the reversal is the entire demand story for theatres here. Tertiary capacity has been built locally, single-specialty centres have followed the residential corridors outward, and a large insured and employer-covered working population has made elective surgical volume worth capturing at home. Two hours of traffic each way to a Mumbai hospital is a real cost to a patient and a real leak to a Thane hospital. Closing that leak requires theatres that can hold accreditation and satisfy insurer and corporate empanelment on the same evidence a Mumbai tower produces — measured air change rates, particle counts, pressure differentials and microbial sampling, not a visual impression of cleanliness.

The result is a market with a mixed brief. Some of it is conversion work, where a 1990s nursing home in Naupada or Panchpakhadi is being rebuilt into a multi-specialty facility. A larger share than in Mumbai is genuinely new construction or a vertical expansion wing on an existing campus. Both are normal here, and they are priced and programmed differently.

What building in Thane actually involves

Land economics are the reason the two differ. Thane is not cheap, but it is not island-city Mumbai either, and that gap is enough to make new floor plate viable rather than forcing every project into a strip-out of a live tower.

On a new build the constraints that dominate Mumbai simply lift. Slab-to-slab in recent Thane construction is commonly 3.6-4.2 metres, which comfortably accommodates a full-depth pressurised laminar plenum with H14 terminal housings in a 900-1200 mm ceiling void above a 2.9-3.0 metre finished ceiling — the configuration you would actually choose, rather than the reduced-depth side-entry plenum a low Mumbai floor forces. A construction hoist or crane is usually available, so panels travel full length instead of being capped near 2.4 metres to pass a passenger lift car, which means fewer joints and a cleaner envelope. There is a lay-down area, so deliveries do not have to be broken into just-in-time night loads. The AHU can arrive as a factory-built skid. Risers, plant deck and service corridors get sized once, to the calculation.

The retrofit half of the market behaves much more like Mumbai, and we treat it that way: knock-down panel sections, sealed hoarding with negative-pressure extraction on the construction zone, a dust and vibration protocol agreed with infection control before the first panel arrives, and core cutting confined to agreed windows because hospital precincts are silence zones. Older slabs designed for 3-4 kN/m² live load need checking before a plenum, an AHU and ceiling pendants are hung from them, and pendant loads need dedicated steel back to the slab.

Coastal air, with room to solve it

Thane sits on the creek, and its summer design condition — roughly 33-34°C dry bulb at 27-28°C wet bulb — is effectively Mumbai’s. The theatre air handling unit is therefore solving a latent load, not a sensible one. Holding 21-24°C at 50-60 percent relative humidity means colder chilled water, a deeper eight to ten row cooling coil, reheat downstream to return supply air to setpoint without adding moisture, and outdoor air pre-treated through its own dehumidification stage instead of being mixed raw into the main coil.

The Thane advantage is not a smaller load. It is that you can usually install the plant the psychrometrics demand. In an island-city retrofit the AHU is frequently sized to the space that survived; here it is sized to the calculation, with a plant deck that allows condenser airflow, service access and future filter changes. Salt aerosol is milder than the Mumbai seafront, but creek air and the industrial belt around Wagle Estate and Bhiwandi still justify epoxy-coated coil fins and protected casings. Monsoon rainfall approaching 2,400 mm and the stack-effect pressure profile in taller buildings both argue for commissioning the pressure cascade in monsoon as well as dry-season conditions.

The room itself

The engineering baseline does not move with the city. General surgical theatres run the NABH and HTM 03-01 minimum of 20 air changes per hour; cardiac, neuro and arthroplasty rooms want 25-30 ACH with a wider laminar canopy over the table. ISO 14644-1 Class 5 is the target in the surgical zone under the canopy, Class 7 at the periphery, with terminal H14 HEPA filters certified to EN 1822. The pressure cascade runs positive from theatre to scrub to sterile corridor to dirty exit at a minimum 2.5 Pa per step, held by pressure relief dampers and hermetically sealed automatic sliding doors with bacterial gaskets and interlocks. The envelope is 50 mm PPGI or SS304 panel with PUF core, closed with self-levelling antibacterial epoxy flooring and coved skirting so there is no floor-wall junction to harbour anything.

Approvals: TMC is not BMC

Any change to built form or floor area routes through the relevant municipal corporation’s building proposal process — Thane Municipal Corporation for the city, but KDMC, Mira-Bhayandar or Ulhasnagar for sites outside it. Fire NOC is issued by the local fire service under the Maharashtra Fire Prevention and Life Safety Measures Act, 2006, read with NBC 2016 Part 4; for theatre work the questions concern panel core surface spread of flame classification, sealing of penetrations between fire compartments, and medical gas zone valve provisioning consistent with NFPA 99. Facility registration sits with the corporation’s public health department under the state nursing homes framework. Fixed imaging in a hybrid theatre adds AERB clearance. MJPJAY and Ayushman Bharat PM-JAY empanelment matters across the Kalyan-Dombivli, Bhiwandi and Ambernath catchment, and empanelment inspections want documented theatre performance rather than a walkthrough.

Delivery from our Pune base

Thane is roughly 145 km from Pune, three to three-and-a-half hours by expressway and then either the Thane-Belapur road or the Kalyan-Shil corridor depending on where the site sits. That makes it a same-day survey city and a next-day escalation city. A single-theatre panel set moves as one trailer load, and site supervision stays resident for the installation period.
